How does Form Health telemedicine work?

Telemedicine through Form Health is designed to help you achieve your weight-loss goals through a telehealth platform that is simple and effective for our clients. You can start treatment following easy steps.

  1. Book a Consultation
  2. Complete a medical intake form online.
  3. During consultation, it will determine whether it suits your goals and whether it's medically safe for you to be prescribed medications. 
  4. When the paperwork is completed, an invoice is sent, and after an invoice is paid, medication is shipped from our associated pharmacy.

How often will I receive my medication?

**Semaglutide and Sermorelin** You will receive your medication monthly. Invoices will be sent each month, and once paid, your medication order will be processed for fulfillment.

**Methylene Blue:** Available options include 30-day, 60-day, and 90-day supplies.

**NAD+ and Testosterone Replacement:** Supply for these medications depends on individual dosing needs.

How much does it cost?

Semaglutide is available through membership pricing, ranging from $350 to $375 monthly. If a medication order from the previous month is skipped, a fee of $95 will be charged.

Sermorelin costs $299 monthly membership.

NAD+ is priced at $499 per 1000mg vial. The supply duration of one vial depends on the injection dose and frequency, with a maximum dose and frequency averaging a six-week supply.

Methylene Blue Capsules are priced based on dosage and supply.

The pricing covers everything you need: monthly follow-ups, medication, supplies, and shipping.

Please note that the cost of Testosterone Replacement Therapy can vary, as it includes other tests and services.

Payment Options

Form Health requires a card on file for billing; statements are auto-billed. It can be canceled at any time. 

What if I need to cancel?

Send an email to formhealth1@outlook.com or contact Form Health directly. There is no contract signed locking you into treatments. 

Note: If medication has shipped, you will be responsible for the total amount of that supply.

What state is Form Health offering services to patients?

Form Health operates and services only Kansas residents.

What if I choose not to join after my appointment with?

There is absolutely no obligation! Any forms completed before or after consultation will be stored in the patient portal, so if you would like to proceed with services later, they will already be on file. 

How will I receive my medication?

We collaborate with compounding pharmacies to fulfill your prescription. Your medication will be shipped directly from the pharmacy to your registered address.

Does Form Health accept insurance?

Unfortunately, insurance is not accepted. Form Health is a cash based clinic.
